Requirements for Podiatrists

  • Proof of successful completion of the American Podiatric Medical licensing Examination Part III formerly known as the PM Lexis III
  • Proof of APMA accredited one-year residency
  • Official transcripts from all colleges
  • Letters of verification from all hospitals and surgical centers where you have had privileges within the last five years
  • 2 passport photos of yourself
  • 2 completed fingerprint cards (included in application packet)
  • Signed authorization to have State and Federal background checks conducted (included in application packet)
  • Completed application (included in application packet)
  • cashiers check in the amount of $38.25 made payable to the Nevada Dept. of Public Safety; for conducting State and Federal background checks
  • cashiers check in the amount of $600.00 made payable to the Nevada State Board of Podiatry for licensure; $100.00 of this fee is non-refundable

    Licensing Application & Renewal Information

    NOTE: Every person who manufactures, distributes or dispenses any controlled substance within this State must obtain a controlled substance license. You need to apply separately for this license to the Nevada State Board of Pharmacy (775-850-1440), 555 Double Eagle Court, Suite 1100, Reno, Nevada 89511-8991.
